Autologous Conditioned Serum (ACS)
Meijer et al. devised a biologic therapeutic preparation known as autologous conditioned serum (ACS) ,a medical device (a syringe) used to produce ACS enriched with anti-inflammatory cytokines. The production of ACS rich in IL-1RA using the new technique is characterized by incubation into a syringe of 50–60 ml of venous blood. After incubation for 24 h at 378C, the blood is recovered and centrifuged. This process stimulates a rapid synthesis of IL-1RA, the concentration of which increased 140-fold, of other anti-inflammatory cytokines (IL-4, IL-10) and GFs (IGF-I, PDGF and TGF-b) that are slightly induced by treatment with glass beads. In addition, other authors observed that, during incubation, the IL-1RA concentration steadily increases for 24 h, possibly also through de novo synthesis, stimulated by the interaction between the glass surface and the blood cells. No significant alterations in protein composition and no significant increase of pro-inflammatory mediators IL-1b and TNF-a were noticed. The autologous serum, now enriched in IL-1RA, IL-4, IL-10 and several GFs, is returned to the patient. Since 1998, ACS, has been used in orthopedic patients and in experimental animal models.

The BLACK PEARL TECHNOLOGY (BPT) is a novel method for preparation of an autologous serum enriched in growth factors and cytokines. The technology is innovative method comprising the use of a syringe consisted of glass beads coated by nano-carbonious materials, BPT is capable to stimulate the cells. It has been proven in laboratory studies that human or animal mono-nuclear blood cells like monocytes and platelets are induced to synthetize and release high concentrations of growth factors and regenerative cytokines when treated by BPT for a few hours.
Why Using BPT

On the other hand, BPT leads to synthesis of interleukin 1-receptor antagonist (IL1-Ra) molecule, a powerful anti-inflammatory cytokine. This indeed, is an inhibitor for interleukin 1 reactions as a receptor antagonist. Interleukin 1 is a key factor for tissue destruction and cell death in body tissues such as cartilage defects in osteoarthritis, skin cell death in wounds, inflammatory endometrial environment in endometrial related infertility and a major contributing factor for aging process. Blocking interleukin 1 through IL1-Ra, may eventually reduce tissue destruction and inflammation in an injured body part. None of the available PRP kits in the market contains IL1-Ra.
